PARENT/GUARDIAN RELEASE: The Parent/Guardian by registering a child hereby gives consent for the named child to participate in the Suffern Girls Youth Lacrosse program. By completing the online registration process the Parent/Guardian assumes all risks and hazards incidental to such participation, including transportation to and from activities of the program; and the undersigned does hereby waive, release, absolve, indemnify and agree to hold harmless Suffern Girls Youth Lacrosse, Inc., the organizers, officers, directors, sponsors, supervisors, participants from any claim arising out of any injury to the player, except to the extent covered by accident and liability insurance provided by U.S. Lacrosse.
INJURY POLICY: The Parent/Guardian understands and agrees that if a child is injured during a lacrosse game or practice, said child will not be allowed to be participate in any further games or practices unless and until a written note is provided from a medical provider that completely clears the child to play.
REFUND POLICY: Refunds for the Spring season will be allowed as per the following guidelines:
Prior to stated close of registration: 100% of registration fee
